Paco nails it:  we have a bureaucracy that we, as citizens, cannot trust to do the right thing.  They abuse their power in ways that let them cover up their malfeasance.  Even when there are rules in place specifically to prevent these types of things, they still happen.  The only rule which really works is not to allow the bureaucracy to get into a position in which they can violate the public trust.
